Mandatory physical and vision fitness certificate issued by a registered medical practitioner for DL applicants aged 40+ or transport vehicle drivers.

How to Fill Form 1A Correctly

Must be signed and stamped by a registered medical doctor (MBBS or higher).

Doctor's medical council registration number is mandatory.

Doctor can also upload Form 1A directly via Sarathi Doctor Portal.
